Documents for connection

1. A copy of the Registration Certificate of Private Entrepreneur (plus copy of the passport for private entrepreneurs) or a copy of the Business Registration Certificate.
2. Copy of the VAT number.
3. Application addressed to the director of Technical Centre Radio Systems Ltd. Kostrubin O.I. where you should indicate:
   • address where connection is carried out;
   • the monthly subscriber fee and the speed (for connection to the Internet-line) you have chosen;
   • bank account details;
   • taxation system;
   • full name of the director (fully written in Ukrainian);
   • the director acts on the basis of: statute, regulations etc.;
   • people in charge, their phone numbers (to solve financial and system-related question concerning the agreement about provided Internet networks);
   • contact telephone numbers.
